1. What is the mission of Dr. Erik Mondrow’s personalized medical practice?
Dr. Mondrow will strive to provide the highest quality medical care, emphasizing a proactive, comprehensive approach to disease prevention and health management, and to offer more time at each office visit to address all of his patients’ concerns.
Dr. Mondrow wants you to be completely satisfied with every aspect of your care.
2. How is Flatiron Premier Medicine different from medical offices?
In his old practice, Dr. Erik Mondrow would often see 20 or more patients a day. In his new concierge practice, Dr. Mondrow will only schedule 7 or 8 patients a day. This means that appointments will start promptly and he can spend more time with you.
If a problem requires extra time for evaluation, Dr. Erik Mondrow can accommodate you. He will offer enhanced access to his personal cell phone and through email. The staff at Flatiron Premier Medicine will assist you in scheduling testing outside our office, and, as your personal health advocate, Dr. Mondrow will assist you in coordinating care with hospitals and specialists.

3. At what hospital is Dr. Mondrow on staff?
Dr. Erik Mondrow is on staff at Avista Adventist Hospital in Louisville, where he is also currently a member of the Medical Executive Committee.
4. Do I still need health insurance if I enroll in Dr. Mondrow’s concierge practice?
Yes. Dr. Erik Mondrow’s personal physician program does not take the place of general health insurance coverage.
His practice is a primary care medical practice, not a health insurance program. You are advised to continue your participation in your health insurance and/or Medicare programs while enjoying the additional benefits of the concierge medical services.
5. Will my insurance still be billed for my office and hospital visits with Flatiron Premier Medicine?
Yes. Your insurance company will be billed directly for office visits with Dr. Erik Mondrow. If your insurance carrier/plan requires a co-pay, Dr. Erik Mondrow is required to collect it.
Office visit charges, with the exception of your Annual Comprehensive Medical examination, are not included in your annual fee.

6. May I get care at Flatiron Premier Medicine if I have insurance through Kaiser or if you are not a ‘Provider’ on my insurance plan?
Yes. If Dr. Erik Mondrow is not a provider for your insurance plan, he will attempt to refer you to “in-network” physicians for any necessary consultations and to “in-network” facilities for diagnostic tests and hospitalizations as medically indicated.
Those services will then likely be covered according to in-network fees. Flatiron Premier Medicine’s office fees may be adjusted to compensate for any increased cost you may incur if he is not able to remain in-network.
7. Is Dr. Erik Mondrow’s concierge medicine practice a Participating Provider for Medicare?
Yes. Dr. Erik Mondrow is a Participating Provider for Medicare.
He will bill Medicare, as well as your supplemental insurer, for covered services on your behalf.
8. Does Flatiron Premier Medicine bill Medicare for the annual fee?
No. The annual fee of Dr. Erik Mondrow’s concierge practice only includes services that are NOT covered by Medicare (or any other payor) and, as such, cannot be paid for or reimbursed by Medicare.
Dr. Mondrow will bill Medicare for your sick visits and for any additional services performed that ARE covered by Medicare. The Annual Comprehensive Medical Assessment is included in your annual fee as are many other amenities.
(Please note there is an exception for the “Welcome to Medicare” one-time physical exam.)
9. What about lab, x-ray, specialists’ fees and hospitalization?
Your annual fee covers membership in Flatiron Premier Medicine, your Annual Comprehensive Medical Assessment exam, and many other amenities listed.
Please contact Dr. Erik Mondrow’s office to get a detailed list of services and amenities at 303.327.4971. All procedures and services not performed in Flatiron Premier Medicine’s office will be billed by the performing entity.
10. What if I have an emergency?
If you have a life threatening emergency, call 911 immediately.
Dr. Erik Mondrow will assist in the coordination of your emergency care thereafter. You may also contact him directly on his mobile phone at any time of the day or night.
11. What if I become ill while traveling or away on an extended vacation?
Call 911 if you have a life-threatening emergency.
Also, feel free to call Dr. Erik Mondrow at any time, as he may be able to provide some assistance over the phone. If you need to see a doctor while traveling, he may be able to help you find a resource.
If you seek care in an emergency room or urgent care center out of your area, you may ask the doctor seeing you to call Dr. Erik Mondrow at any hour for any assistance he can offer.
If you should require hospitalization while away, he will, at your request, attempt to establish daily phone communication with you and your attending physician(s) to ensure continuity of care. Also, Flatiron Premier Medicine will be happy to provide you with an electronic version of your important medical information for you to take with you whenever you travel!
12. What if I need to see a specialist or a surgeon?
As always, Dr. Erik Mondrow’s patients are free to see any specialist they wish.
He is available to help decide which specialists to see and to coordinate such consultations. In this way, the most appropriate resource is used, the earliest arrangements are made, and your applicable medical information is sent in advance of your specialist visit.
13. Is Dr. Erik Mondrow’s personalized medical practice accepting new patients?
Yes, Dr. Erik Mondrow is currently accepting new patients. By design, Dr. Mondrow’s concierge physician practice is a membership practice with a limited enrollment. Once that enrollment limit is reached, a waiting list will be established.
Every effort will be made to accommodate interested patients, but the enrollment limit must be honored in order to continue to provide the highest standard of personalized care and service to all of Dr. Mondrow’s patients.
